The Federal Government has recently revised the race and ethnicity groupings for the federally required Equal Employment Opportunity reports. Consequently, employers with more than 100 employees are required to survey their employees to determine race and ethnicity as defined by the new categories. All employees are asked to complete the Employee Survey on Race and Ethnicity between now and April 16, 2010.

Please be advised that Winston-Salem/Forsyth County Schools is required to complete this information for compliance with federal regulations. In the event you elect not to complete the form, your supervisor will be required to provide the information based on direct observation. For this reason, we strongly encourage each employee to submit the electronic form by the April 16 deadline in order to ensure accuracy of the information reported.

You may access the form through the WS/FCS homepage, selecting the link at the bottom of the page entitled "Employee Survey on Race and Ethnicity" in the News and Events section.

Click on the link within the general information page to be directed to the Voluntary Equal Employment Opportunity Screen.

Select the link for "Voluntary Equal Employment Opportunity Identification Form for Employees".

Complete the form. Don't forget to select "Submit" at the bottom of the form.

The UserID and password are your "i-notes" user ID and password. If you do not have an "i-notes" account, you can obtain your assigned User ID through the User ID Access menu; and your password will be the last 4 digits of your Social Security Number. We encourage you to update this to a unique password for purposes of maintaining your information security.
